Search by City and Date of Death

City Name: Select city name from the list

Date of Death: Enter date of Death

Enter security Code showing on the screen

 

Step 5: Click on Submit and follow instructions

Now the Death Certificate details will be displayed on the Screen. You can see application status. If the certificate is ready, you can download it.
