Punjab Death Certificate Online: Apply, Track, Download & Correct

How to Apply for a Punjab Death Certificate

  1. Open the official Connect Punjab portal or the relevant Punjab Government/municipal service portal for the place where the death was registered.
  2. Register or sign in and search for the required death-registration service.
  3. Select the correct service: copy of death certificate, non-availability certificate, correction, or delayed registration.
  4. Enter the deceased person's particulars and the applicant's details accurately.
  5. Upload or submit the supporting documents required for the selected service.
  6. Submit the application, pay the applicable official charges shown by the portal, and save the application/reference number.

Documents Commonly Required

The exact document checklist varies according to whether the application is for a certificate copy, correction or delayed registration. Punjab's Health Department document list includes proof connected with the deceased and, for delayed registration, supporting evidence of death.

  • Applicant identity and residence proof, where required.
  • Details or proof relating to the deceased person.
  • Hospital/nursing-home record or other recognised proof of death, where applicable.
  • For accidental, suicide or murder cases, post-mortem report and FIR may be required under the applicable checklist.
  • Cremation-ground report, immersion-of-ashes record, Bhog card or Antim Ardas record may be accepted in relevant cases.
  • For delayed registration, additional declaration/affidavit, non-availability certificate and statutory approval may be required depending on the period of delay.

Track Status and Download the Certificate

  1. Open the same official portal through which the application was submitted, or the Punjab eServices facility where supported.
  2. Choose the available Track Status or Download Certificate option.
  3. Enter the application/reference number and other requested details.
  4. If the application is approved and an electronic certificate is available, download the official copy and retain it securely.

Current Law on Death Registration

Death registration in India is governed principally by the Registration of Births and Deaths Act, 1969, as amended by the Registration of Births and Deaths (Amendment) Act, 2023. The 2023 amendment came into force on 1 October 2023.

  • The amended framework expressly provides for digital registration and electronic delivery of birth and death certificates.
  • The law strengthens the registration system and grievance-redressal framework.
  • The amended Act also contains provisions connected with medical certification of the cause of death.
  • The Model Registration of Births and Deaths Rules, 1999 were subsequently amended as the Model Registration of Births and Deaths (Amendment) Rules, 2024 for States and Union Territories to frame corresponding rules.

Delayed Registration of Death

A death should be reported within the statutory period. When registration is delayed, additional fees, documentary proof, declarations and approval from the prescribed authority may become necessary depending on how long the registration has been delayed. Applicants should use the current Punjab delayed-registration form and follow the instructions applicable on the date of application.

Correction in a Punjab Death Certificate

If an issued certificate contains an incorrect name, date, address or other entry, use the official Correction in Death Certificate service rather than submitting a fresh ordinary certificate request. The registrar may require documentary evidence supporting the requested correction before the register can be amended.

Before You Submit

  • Apply through an official Punjab Government or competent municipal portal.
  • Use the place of registration of the death to identify the correct local registrar or municipal authority.
  • Check the current documents and fee shown by the official service before submitting.
  • Keep the application/reference number until the certificate is issued.
  • For legal, inheritance, insurance, pension or property purposes, ensure the certificate particulars exactly match the underlying civil-registration record.
